The Challenge

Health data is scattered across portals, PDFs, faxes, labs, and wearables. Patients are unable to identify patterns or understand what is important, which hinders their ability to proactively manage their health.

Our Solution

YHYP is an AI-powered health intelligence platform that translates fragmented data into clear insights, trends, and personalized plans to power proactive health management.

YHYP Dashboard APP

What is it, and for whom?

YHYP is a B2B SaaS Model, that will be offered as a service to Concierge & Preventive Care Practices (not directly to patients).

Practices pay a monthly fee for:

  • AI-enhanced clinical insights
  • Automated data aggregation (labs + wearables + history)
  • Physician dashboard + patient reporting tools
Actionability

Simple Daily Health Plan

YHYP turns doctors' recommendations into clear, doable daily plans that users can understand at a glance and act on with confidence. Each completed plan can be checked off, helping users stay accountable and see steady progress without extra effort.

Synthesis

Your Health, Simplified

YHYP consolidates health data from multiple sources and applies AI analysis to interpret it as a whole. Instead of showing everything at once, this summary view highlights the most important health signals, helping users quickly understand what matters before diving deeper.

Understanding

Meaning Behind the Numbers

YHYP allows users to tap into individual health metrics and see clear explanations of what each value means for their health. By placing each indicator in the context of other data and lifestyle factors, YHYP helps users understand patterns rather than isolated numbers.

Research

Our research combined market analysis, competitor audits, and user and expert interview.

Problem Statement

How might we turn fragmented, unstructured health data into a unified, intelligible system that supports proactive care for both patients and physicians?

Design Iteration

From information to action

Actionable plans are suggested by AI but reviewed and approved by physicians. On the patient side, we designed lightweight streak and progress mechanisms to support consistency.
From information to action

Data is not the key, trends are

Raw data is never the starting point of the experience. We organized the system around trends, baselines, and deviations, so users can answer a higher-level question: Is my health moving in a good direction?
Trends visualization

AI as an interpreter, not a decision maker

AI does not give diagnoses or final judgments. Instead, it translates complex data into plain language insights and highlights questions that users should discuss with their doctors.

Value

The System Patients and Doctors Have Been Asking For

How We Are Building

Inspired by Gabe and the Forefront team, YHYP is built around these radical ideas:

  • One Integrated Health Story
  • Makes the Invisible Visible
  • A Personalized, Proactive Plan
  • A Second Set of Eyes for Physicians

Why This Matters

Key Benefits for Patients & Physicians

Physicians: Reclaim Time & Confidence

  • Unified timeline eliminates hours of manual chart review with instant patient history access
  • AI-driven summaries accelerate accurate documentation
  • “Second set of eyes” spots hidden risks for earlier intervention

Patients: Clarity & Motivation

  • Understand your health status and changes with a board level report
  • Track tangible progress toward your health goals
  • Take control with personalized plans for long-term health

Insights from Physicians and Patients

User Research Validation

“The summary and report give me instant context and catch patterns I’d otherwise miss.”

-- Patient S

“I like that it helps me focus on what’s actually important, instead of digging through everything myself.”

-- Patient D

“I love having vitals, labs, and wearable data all in one place. I’d pull this up with patients and walk through it visually during visits — incredibly helpful for clinical decision-making.”

-- Dr. Sheena

“From a cardiovascular standpoint, these AI recommendations are appropriate. Doctors often miss things like ACE inhibitors or the DASH diet — having AI surface those and letting me click once to add them to the action plan would be a real one-stop shop.”

-- Patient D
